Claude e99b5781c8 fix(core): replace persisted ContinueSession sentinel with real agent ID ...

CompareAndSetAgentSessionID only fills an empty ID; after restart the
stored sentinel __continue__ would block writing the actual session id
from the running agent. Handle that case explicitly.
