* feat(discord): add mode and reasoning button controls

Use Discord buttons for /mode and /reasoning, and apply Claude Code mode changes to the current session when supported.

* fix(discord): clean up mode labels and log button fallback errors

Remove the extra mode marker symbol, show the current mode with text instead, and include the underlying error when permission button fallbacks trigger.
