refactor: 统一 providers 层，提取共享模块消除重复代码
- 提取 shared/auth.ts、channel-resolver.ts、route-policy.ts、provider-registry.ts
- 提取 shared/openai-stream-utils.ts 和 responses-stream-utils.ts
- 统一 index.ts 为 UnifiedProxyEndpoint（303→55行）
- 各 provider 文件精简约55%，只保留 buildProxyRequest
- 修复 claude-to-openai-proxy.ts 类型错误
- 添加 E2E mock 测试基础设施（mock-upstream + setup + run-e2e）
